Physicians
We are looking for physician leaders who are engaged and present to collaborate with the facility care team to ensure we are delivering patient focused and quality care for our Community Partners. This is a unique opportunity for a high performing, driven, caring, primary care physician with the ability to work in a collaborative care team environment.
Nurse Practitioners and Physician Associates
The ideal candidate will have long term care experience; however, extensive clinical experience in Family Practice, Internal Medicine or Hospitalist may be adequate for a motivated individual. For those with clinical experience, but no long-term care experience, we provide training regarding the regulatory and clinical model of long-term care.
Pain Management Nurse Practitioners & Physician Associates
The ideal candidate will have experience meeting the pain management needs for older adults. For those with clinical experience, but no long-term care experience, we provide training regarding the regulatory and clinical model of long-term care.
